1) Create an online Personal Health Record
2) Request non-urgent appointments
3) Medication refill requests
4) Request certain laboratory results
5) Request referrals
6) Communicate securely with the office staff

What is a Personal Health Record?
A PHR allows you to organize and store your medical history, such as what medications you are taking, any allergies you may have, immunizations and family history.
Who can use the RelayHealth service?
Any established patient who sees a doctor that is using RelayHealth may use the service.
How do I know if my doctor uses RelayHealth?
Visit www.relayhealth.com and click Find Your Doctor to view the list of participating providers.
How do I select a physician who uses RelayHealth?
Visit www.relayhealth.com and click Find Your Doctor to view the list of participating providers. To use RelayHealth, you must have an existing relationship with a physician using RelayHealth. If you you want to change to a new physician who uses RelayHealth, you must schedule an in-office visit before communicating with them online.
Is my privacy protected?
Unlike e-mail, a Relay™ message uses secure technology. Only you, your doctor, and your doctor’s authorized staff have access.
