Flu Shot & Flu Mist Clinics - September-December 2011

We will be offering flu shot clinics for our established patients ages 6 months and up, on three Saturdays this fall to make it easier to get your child vaccinated against this season's influenza.

Several clinic dates are now full - please check the schedule below to see which dates are available.

We will be unable to accommodate any siblings of patients unless they also already have an appointment scheduled at the same time. We will only see patients who already have a pre-booked appointment.

Questionnaire

Please download and complete the flu vaccine questionnaire now and have it available to give to our staff when you arrive.

Each patient must have their own flu vaccine questionnaire completed prior to their appointment.

If any of the answers are “YES” on the questionnaire, your child may not receive the flu vaccine as part of our flu shot clinic and will be required to schedule a regular appointment during normal business hours with their primary care physician prior to receiving the vaccine. Your child is still eligible to receive the flu vaccine but will need an appointment with their doctor.

Payment

For your convenience, we will not collect a copayment at the time of service during weekend flu vaccine visits. We will however be sending you a bill by mail for the amount of the copayment, co-insurance or applied deductible due if required by your insurance.

Appointments

At this appointment your child will be given the flu vaccine (either FLU-MIST or injectable vaccine) by a member of our medical assistant staff, as ordered by their physician.

Your child's primary care physician will have reviewed his or her records to be sure they qualify for the flu vaccine, and in which form. Your child will not be seeing a physician as part of the weekend flu vaccine clinic.

You may schedule your child's appointment beginning August 1st by calling our office at 510-792-4373. We will have a limited number of weekend appointments available so please contact us soon. We will also have appointments available with your child’s doctor during regular business hours, Tuesday through Friday.

• Tuesday, November 29, 4:30 - 6:00 pm
• Thursday, December 1, 4:30 - 6:00 pm

Depending on the needs of our patients and their families we may offer or add additional flu shot clinics during the months of December 2011, through February 2012.
